Blended Learning Program Registration Steps

1. Prepare Administrative Documents
   • Softcopy of High School/Vocational School/Islamic Senior High School/Package C/Equivalent Diploma (legalized)
   • Softcopy of SKHUN/NEM/UN/Transcript (legalized)
   • Softcopy of Formal Photo (red/blue background)
   • Softcopy of ID Card, Birth Certificate, and Family Card
   • Proof of entrance exam payment IDR 200,000
2. Download and Fill Out Registration Form
   The form can be downloaded through the following link:

3. Send Documents via Email
   All documents should be sent to blendedlearning@ppmschool.ac.id
   with email subject:
   “SMB Blended Learning Registration – [Full Name]”
4. Make Entrance Exam Payment
   Transfer the registration fee of IDR 200,000 to the following account:
   • Bank Mandiri: 1030085288583 a.n. Yayasan Pendidikan dan Pembinaan Manajemen
   • Bank BCA: 6860038801 a.n. Yayasan PPM
5. Save the transfer receipt to be attached with the registration documents.
